Famoxim

Famoxim (Famotidine) is used for the treatment of duodenal ulcers, prevention of their recurrence, benign gastric ulcers, Zollinger-Ellison syndrome, and mild to moderate esophagitis. This drug reduces stomach acid secretion by blocking H2 histamine receptors.

General Guidelines

This medication has been prescribed for the treatment of your current condition. Do not use it for similar conditions or recommend it to others. Before taking this medication, inform your doctor or pharmacist about the following:

  • Any allergies

  • Use of other medications

  • Pregnancy and breastfeeding

  • Presence of other medical conditions

Indications

Dosage and Administration

Before starting this medication, inform your doctor or pharmacist about any other medications, herbal products, or supplements you are using.

This drug is taken orally and can be administered with or without food.

The exact dosage is determined by your doctor, but general guidelines are as follows:

Adults

  • Duodenal ulcer: The recommended initial dose is 45 mg of famotidine, to be taken at night. Generally, healing occurs in most patients within 4 weeks. If endoscopic examination shows that the ulcer has healed, this period may be shorter. In patients whose ulcers have not healed during the 4-week period, treatment should continue for an additional 4 weeks.

  • Prevention of duodenal ulcer recurrence: The recommended dose is 20 mg of famotidine at night.

  • Benign gastric ulcer: The recommended dose is 40 mg of famotidine at night. The treatment period should last between 4 to 8 weeks unless faster healing is confirmed by endoscopy.

  • Zollinger-Ellison syndrome: Treatment starts with 20 mg of famotidine every 6 hours and is then adjusted based on individual response. Doses up to 80 mg per day have been used for up to one year without significant side effects or tachyphylaxis.

  • Mild to moderate esophagitis: The recommended dose for mild esophagitis is 20 mg of famotidine twice daily. For mild to moderate esophagitis, the recommended dose is 40 mg twice daily. The usual treatment period is 6 weeks; if the condition has not improved, treatment should continue for an additional 6 weeks.

Renal impairment

  • In patients with kidney function impairment (creatinine clearance less than 30 mL/min) and in dialysis patients, the daily dose of famotidine should be reduced by 50%.

  • Famotidine 20 mg tablets should be given at the end of dialysis or immediately afterward, as some of the active ingredient is removed by dialysis.

Elderly

  • Recommended doses are the same as for adults.

Missed dose

  • If you forget a dose, take it as soon as you remember.

  • If it is close to the time of the next dose, skip the missed dose and continue your regular dosing schedule.

  • Do not double the next dose to make up for a missed one.

Pregnancy and Breastfeeding

  • Use during pregnancy is not recommended and should only be prescribed if necessary.

  • Famotidine is excreted in breast milk; therefore, breastfeeding mothers should either discontinue famotidine or stop breastfeeding.

Warnings

Contraindications of Famoxim

  • Patients with a history of allergy to famotidine or any of the ingredients in the medication.

  • This drug is not recommended for individuals with a history of allergy to other H2 receptor antagonists, such as ranitidine or cimetidine.

Warnings and Precautions

  • Patients with malignant gastric tumors should complete appropriate treatment to remove the tumor before using famotidine for gastric ulcer therapy.

  • Since famotidine is primarily excreted through the kidneys, it should be used with caution in patients with renal impairment.

  • Use with caution in elderly patients.

  • For long-term treatment with high doses, monitoring of blood cell counts and liver function is recommended.

  • In patients with chronic gastric ulcer disease, sudden discontinuation of the drug after symptom relief should be avoided.

Drug Interactions

  • Atazanavir, Ketoconazole, and Itraconazole: Absorption is reduced → These drugs should be administered at least 2 hours before taking famotidine.

  • Probenecid: Increases plasma famotidine concentration by 50% → Concurrent use should be avoided.

  • Antacids: May reduce famotidine absorption, leading to lower plasma levels → Famotidine should be taken 1–2 hours before antacids.

  • Sucralfate: Inhibits famotidine absorption → Should not be administered within 2 hours after taking famotidine.

  • Hemodialysis patients: Concurrent use with famotidine may reduce the effectiveness of calcium carbonate.

Side Effects

Like all medications, this drug may cause unwanted effects alongside its therapeutic benefits, although not all side effects occur in every individual.

Common side effects include headache, dizziness, constipation, or diarrhea.

If any of these side effects occur, inform your doctor or pharmacist immediately.

Storage Conditions

  • Store the medication at a temperature below 30°C, away from light and moisture.

  • Keep the medication out of reach and sight of children.

  • Take the medication exactly as prescribed by your doctor.

  • Complete the full course of treatment.

  • Sale without a prescription is prohibited.
